Medical Forms for Summer Events 2020

The new year means new medical and shooting forms.

Note that there are three different parts to the medical form. 

  • Part A is an informed consent, release agreement and authorization that needs to be signed by every participant (or a parent and/or legal guardian for all youth under 18). Please sign the form.
  • Part B is general information and a health history. It has 2 sides. Both sides should be filled out. Part B requires the front and back of your insurance card along with your child's immunization dates. Print outs of the immunizations and dates are acceptable.
  • Part C is the physical exam completed by a certified and licensed health care provider. Part C is required for overnight trips over 72 hours.

The form is valid through the end of the 12th month after the date it was administered by your medical provider. For example, if you got your physical on June. 3, 2019, it’s valid until June 30, 2020.

When you have completed forms A B and C, have a current list of immunizations, and the front and back sides of your 2020 insurance card, please make two complete copies. The shooting form attached requires FOUR (4) signed copies. If you want your son to be able to participate in shooting sports at camp, please print and deliver 4 copies.
 
I will not accept digital copies emailed to me.  Please print 2 complete copies of the medical forms, 4 copies of the shooting form, and deliver them or snail mail them to me at 307 Channing Road in Burlingame.

  
PARENTS - If you are going to summer camp, you are required to have a current certification for the YPT course. If you are staying LESS THAN 72 hours, camps require form A & B for you.  If you are staying more than 72 hours in a 7 day period, camp also requires Form C.
 
Check List
Form A - parent signature and date
Form B - two pages need to be filled out.  This form asks for a list of current immunizations and the front and back of the current year's insurance card.
Form C - Doctor's signature and date.
Snail mail or deliver TWO (2) copies of each form, immunizations, and insurance card to me. Do not email me electronic forms. I will not print them out for you. I will return the email to you.
PS - don't forget 4 copies of the shooting form if you want your son to be able to shoot at camp.

Memorial Day Flag Placement at the Golden Gate National Cemetery

Please join Troop 28 Scouts (lead by Scout, Johnny Willey) for the Memorial Day Flag Placement at the Golden Gate National Cemetery

Time: 8:00 a.m. - Meet-up by the hill with the flag pole just inside the main entrance.

Ceremony 8:30-9am, 9am Flag planting begins. 

Order of the Arrow members will be facilitating the distribution of pokers and flags.  If you are an OA member, even if you plan to work with your unit directly, please bring your OA Sash and register at the OA table before the ceremony begins.

Note: We will go to breakfast afterwards at the Millbrae Pancake House.

RSVP: Please rsvp / or questions by Monday 5/21 to Johnny Willey (jonathanwilley.16@gmail.com; j_sobolik@hotmail.com) so the Troop can register.

More info: The Golden Gate National Cemetery is one of a number of National Cemeteries authorized by the United States Congress in the 1930’s and finished in the 1940’s. Construction began in 1937 with the first internments in 1941. Golden Gate National Cemetery is built on the former location of Jersey Dairy, which was owned by the Sneath Family.

California Attorney General, Earl Warren, later governor of California and Chief Justice of the United States, was the keynote speaker at the opening ceremony. Golden Gate National Cemetery is one 131 National Cemeteries located in 39 states. The cemetery grounds are the final resting place of over 137,000 servicemen and women, set on 161 acres.

As befitting the final resting place of our nation’s valiant servicemen and women, please ensure that all members of your group conduct themselves in a dignified and reverent manner. No running is allowed. Park in designated areas only. Obey all signs and follow all traffic instructions. No bicycles or pets are allowed onsite (service animals excepted).

Tips:
• Establish a Central Meeting Place for your group
• Carry your Unit Flag so that you can be easily spotted or identified
• Use FRS/GMRS frequency radios to communicate with each other
• Establish a protocol if you or your group gets LOST or SEPARATED
• Bring a small Snack and a Water Bottle (Practice “Leave No Trace!”)
